• Include the name of the institution or university you are now attending or plan to attend after high school, along with their mailing address for the financial assistance office.

• Please include a headshot photo of yourself that is at least 400 pixels wide in the.JPG or.PNG file type.

• Your entry could be rejected if we don't get all the necessary details by the deadline.
